BEFORE YOUR PROCEDURE

Admission & consent forms

Complete your admission information and Patient Information & Consent Form through the A1 pre-admit portal.

For IV sedation admission forms, select Other Surgical Procedure as directed by our team. Keep your appointment-specific instructions alongside your forms.

Open pre-admit portal

Opens the external pre-admit system in a new tab.

If you cannot sign in, cannot find the form requested or are unsure which procedure to select, call our team. Keep the latest instructions supplied for your appointment; completing a form does not confirm your procedure or suitability for sedation.

Prepare for your procedure

Keep your medical history and medication list up to date. Follow the fasting, medication and carer instructions provided for your planned care, and let our team know about any health changes.

INFORMATION FOR YOUR DISCUSSION

Risks & complications.

Your dentist and anaesthetist explain the risks relevant to your procedure, medicines and health. This overview supports that discussion; it does not replace your individual consent information.

IV sedation

The depth of sedation can vary. Deep sedation can affect breathing and heart function, which is why individual assessment, monitoring and recovery arrangements are important.
